Refers individual served or family to community resources to assist in recovery from mental or physical illness and to provide access to serves such as financial assistance, legal aid, housing, job placement or education. Monitors employee service productivity and implements plans of action as needed. Ensures employees are informed of and follow all policies and procedures Provides training to employees to enhance service provision and implementation as needed. Ensures all employees receive initial training and complete all annual/on-going training as required. Performs clinical assessments of the behavioral health needs of individuals presenting for services. Organizes support groups or counsel family members to assist them in understanding, dealing with, and supporting the individuals served. Monitors, evaluates, and records individual’s progress according to measurable goals described in treatment and care plan. Identify environmental impediments to individuals served of their progress through interviews and review of individual’s records. Counsel individuals served in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life. Communicates with other Program Supervisors on various services and needs to enhance the organization and services to individuals. Establish and nurture on-going collaborative relationships with the local Hospitals (eg ECRH), the ACT Team, CST, CSU, and other referral sources.į. Facilitate weekly team meeting and clinical supervision.ĭ. ![]() Ensure daily operations of the program and services abiding by the Service Guidelines of DBHDD Provider Manual for services (Refer to the DBHDD Provider Manual)ī.
